Insurance
Madison County Hospital provides friendly customer service
personnel to help answer questions about your hospital bill. We will do everything
we can to assist you with questions about your insurance coverage, but we cannot
guarantee payment or policy benefits. To help us assist you, please bring a copy
of your insurance card when you arrive at the hospital for treatment. You will
be asked to assign your insurance benefits. This will allow your insurance company
to send the payment directly to the hospital. You should familiarize yourself
with the terms of your insurance coverage and call your insurer with any specific
questions. This will help you understand what to expect and to plan accordingly.

HMO/PPO
If you are a member of an HMO or PPO, your plan may have special requirements,
such as a second surgical opinion or pre-certification for certain tests or procedures.
It is your responsibility to make sure the requirements of your plan have been
met. If your plan's requirements are not followed, you may be financially responsible
for all or part of the services rendered in the hospital. Some physician specialists
may not participate in your health care plan. If not, their services may not be
covered.
Medicare
If you are covered
by Medicare, bring your Medicare and supplement insurance cards. If your Medicare
coverage is through a Medicare Choice plan, otherwise known as a Medicare managed
care plan, please let the registrar know and provide that card. You should be
aware that the Medicare program specifically excludes or limits payment for certain
items and services, including cosmetic surgery, some oral surgery procedures,
personal comfort items, and hearing evaluations. Deductibles and co-payments are
your responsibility.
Medicaid
If
you are covered by Medicaid, we will need a copy of your Medicaid card. Medicaid
has payment limitations on a number of services and items. Medicaid does not pay
for the cost of a private room unless medically necessary. Newborns must be enrolled
in Medicaid. Notify the hospital of your newborn's identification number as soon
as possible.
No Health Insurance
If
you do not have health insurance, you will need to provide some identification
to verify your address. A representative from the hospital credit department will
discuss financial arrangements with you. The representative will assist you in
applying for Medicaid. Depending on your income level, you may qualify for full
or partial payment write offs for medically necessary hospital services. Ask the
credit representative for details.
Your Hospital Bill
Madison County Hospital will submit bills to your insurance
company and make every effort to assist you in getting your bill paid. Please
understand, your insurance policy is a contract between you and your insurance
company. The hospital has no legal authority over your insurance company. Therefore,
you have final responsibility for payment of your bill. We have several payment
options available.
Bills for Other Professional Services
If you have certain tests or treatments in the hospital,
you may receive bills from physicians you did not see in person. These bills are
for professional services rendered by doctors, such as pathologists, radiologists,
Emergency Room physicians, etc. Insurance companies require them to submit separate
bills. If you have a question about these bills, please call the telephone number
