Patients wishing to request a release of information form for copies of their medical records may contact the Medical Records Department at 740.845.7102 or toll free at 866.357.4677. Patients may also request copies of records in person.

Medical Records Hours:
8:00am–4:30pm, Monday–Friday

A signed release from the patient is required prior to releasing the medical records. Requests for records for continuity of care are processed immediately. In accordance with HIPPA guidelines, all other records are processed within 30 days if the records are on-site. If records are in off-site storage, 60 days will be required for processing. There may be a charge associated with the copies of the records. Please contact Medical Records for further information regarding medical record copy charges.
